I know this would be very difficult to implement so that it accomplished outing the truly bad guys. I recognize that very strick control is needed, I like the idea of 3 or more "uncorrected" incidents. Any more good ideas?

I would have a boilerplate attached to the list. I would not be making any judgements, just reporting incidents within the published guidelines.
