The LVA publisher seems to be a pretty small operation. They are set up for the routine subscriber, but seem to have problems handling the exceptions. The annual subscription is so cheap that I'd recommend that you go with that from the start. That way you also get the big coupon book in addition to the printed coupons in each issue.

By the way, the coupons always indicate the casinos have the right to terminate a special promo offer at any time. This has happened a couple of times and may be the reason for the cancel stamp on the sample magazine coupon since you got it after normal publication date.
