Wow, I just looked at page 4 of The Gaming Table. It shows: "500 Club, 6 S. Missouri Ave, Atlantic City NJ, opened: 1918; closed: yes."

The second Mason card for the chip (in first post of this thread) says: "4 South Missouri Avenue, Atlantic City NJ."

Case closed. Anyone know some details/history of the Club?
