Is it a printing error? I'm no printing expert but I believe each color is printed as a seperate step in the process. Could one of the ink colors have been out at the time this inlay was printed?

If that is the case, I would guess that there are more of them as the inlays are printed in large sheets.

Again, just a theory. I have a $2.50 chip from Las Vegas Casino that has a color missing from the printing. You can see it at:
