Twice in my slot card collecting, I have been at a casino and gone to the club desk for a card, only to have them state they were out of printed cards and I was given a blank white card on which my name was embossed or indented. This was as Old Chicago out west and the Lake of the Torches in northern Wisconsin. Got back to the latter, but never got a real Old Chicago card.
