The Flamingo chip is concentric circles similar to the white one Don has scanned.

USPC and Burt Co both made the plain mold.
TR King also made a plain mold, but I have not seen one with CC's.
If I had to guess, I would guess that Burt made all the scanned chips in this thread so far.
