Greg, the printing may have been the problem here. I am not sure how chips are produced, in fine details, so I am guessing.
Is the printing done directly on the chip, or is the printing done on paper or plastic sheetes that are in some way bonded to the chip itself? In either case, problems in the printing process could cause colors to not show up where they should, or, as in the yellow area, it prints color over the entire circle instead of just where it is supposed to be. (This is known as the color "bleeding" out from the desired area.
If the printing is not the cause of the problem, it may be the chip was exposed to some kind of chemical liquid or vapor that reacted with the inks to change color.
Again, I'm only speculating here, in case no one else has solid information for you.
